chore(claude): add unknowns-discovery phases to /plan and /lfg

Summary

  • /plan gains a new "Surface the unknowns" phase (Phase 3, renumbering the old Phase 3 → 4 and Phase 4 → 5): a blindspot pass over open decisions/edge-cases/no-precedent territory, followed by a one-question-at-a-time AskUserQuestion interview prioritized by blast radius. Answers are recorded in the plan's Decision section as Settled in interview: bullets the executor must not re-litigate. AskUserQuestion was added to the command's allowed-tools.
  • /lfg gains a deviation log: implementation-notes.md at the repo root, written the moment the plan and reality diverge (deviations / discoveries / judgment calls), folded into a mandatory ## Deviations & judgment calls PR-body section and then deleted.
  • /lfg gains a comprehension close-out (new Phase 8, after Commit & PR): the final message must surface the 3–5 non-obvious decisions (leading with the deviation log) and three merge-gate questions the user should be able to answer before merging.

Why

Catch unknowns before execution (discovery) instead of after (review-fix loops); prose-only change to .claude/commands/.

Test plan

  • Diff limited to .claude/commands/plan.md and .claude/commands/lfg.md
  • Phase headings renumbered sequentially in both files (verified via grep)
  • Markdown reviewed by hand for broken formatting
