docs(changelog): cut 1.3.0 and date the 1.2.x section

Summary

Cut [Unreleased] into a 1.3.0 section (today's four features #92–#95 and the link-scheme fix #100) and give the 1.2.0/1.2.1 block its own heading. The release kit never edits the changelog, so the 1.2.x content had stayed under [Unreleased] through both releases.

Prepares bin/release minor → 1.3.0.

Test plan

  • Only CHANGELOG.md changes; every entry kept verbatim, moved under the version it shipped in (checked against git diff v1.2.1 -- CHANGELOG.md)

Deviations & judgment calls

  • The 1.2.0 and 1.2.1 releases were cut the same day (2026-09-30) and share one changelog block, so the heading says so instead of splitting entries by guess.
