fix(report): note truncated agent observations

Summary

  • record observationTruncated: true when any semantic observation in an agent step is truncated
  • cover both agent.act and judgment-based steps (assert, waitFor, and extract)
  • update the report schema, fixtures, SDK type coverage, documentation, and generated skill reference

Using a real built CLI run with a custom engine that returns a truncated semantic observation and blocks before any model call:
