Prefix all models with provider for consistency

We now prefix all known model names with their providers.
This means that we've added the following provider prefixes:
anthropicgoogle-glavertexai->google-vertex
Old model names should still work, ex claude-3-5-sonnet-latest and anthropic:claude-3-5-sonnet-latest should both work.
Also, improving coverage to 100% for models/__init__.py
