Carry of #26682: fix(setup): report actionable harness authentication checks.
Original purpose: provider configuration and protocol failures in cmux setup harness report explicit typed authentication status and recovery actions instead of state: "ok" with no recovery. Signed-out profiles retain profile-specific login commands; verification failures become actionable needs_user diagnostics while setup performs no writes.
This carry preserves the source implementation and tests, and merges current feat-cmux-next to refresh the crash-class/base comparison. #26682 remains the source PR; this PR supersedes it for landing.
No capture: CLI JSON only; no UI surface changes.
