Fixed editing the time off document url field not saving removed url document.

Fixes time-off request document field editing so clearing or changing the URL now saves the change and drops the stale document reference.
Previously, removing the document URL left the old value in the form, so the request kept the stale document. Now the uploader emits the cleared URL, resetting documentUrl to null. When the URL is cleared or changed, documentId is also reset, so the subscriber can't restore the asset's URL on reload. The uploader only emits a checked URL if it still matches the input's current value, so an image fetch that finishes after the field was cleared can't restore the removed URL.
