feat(pstack): propose a cheap always-on signal after a deep debugging dig
Summary
Adds one Pattern bullet to principle-fix-root-causes: when the deciding fact took a deep dig to find, propose the cheap, always-on signal that would have shown it at first look. No version bump; that lands in a separate bump PR.

Overview
Extends the Fix Root Causes debugging skill with one new Pattern bullet: after a long investigation, suggest a cheap, always-on observability signal (logging, metrics, etc.) that would have surfaced the deciding fact immediately.
No plugin version bump in this PR (planned separately). Validation via node scripts/validate-plugins.mjs.
