docs: apply the wording follow-ups from #4277, #4278 and #4279

This PR applies the optional wording items from the merged PRs' final reviews. It changes only docs and comments.

  • ADR 0007 (#4277 review): the device-spec bullet now says the backend echoes the granted device as lease.device.
  • snapshot-route.ts (#4278 review): the 'off-route' plan comment now says it covers every capture with no bridge plan, for example a pinned backend or a custom-actions read.
  • RunnerTests+SnapshotCapturePlan.swift (#4279 review): the MARK points at apple/runner/README.md "Capture plans", where those rules now live, like the SnapshotChannelPolicy MARK.

I did not take the #4277 note about longer doc-path citations in source comments. Those comments now point at the doc that owns each rule.
