Version sql-fs-api 1.1.1 from the four pending changesets (#238, #239, #251, #265).

  • CHANGELOG.md: new 1.1.1 section generated by pnpm changeset:version
  • package.json, README.md badge, src/api/openapi-spec.ts: 1.1.0 → 1.1.1
  • Consumed changesets removed; lockfile unchanged

The 1.1.1 notes include one client-visible change: post-dispatch coherence failures now return EEXEC_OUTCOME_UNKNOWN instead of ECOHERENCE.

Verification: pnpm check passes on a clean checkout.
