Fixes skill-publish conflict errors that said "in this workspace" when skill names are actually unique per creator across the whole account, so users were looking in the wrong place.

The conflicting skill may live in a workspace the project was never linked to. The error now names the account scope, and includes the owning workspace when the 409 detail carries it. This fixes #1349.
