fix: keep v2 Flow skills self-contained
Summary
- remove v2 skill redirects to deprecated v1 skill names
- keep migration warnings self-contained and direct all new work to the v2 engine
This prevents consumers that intentionally exclude v1 skills from emitting broken read_skill routes.

Overview
Makes Relayflows v2 skills self-contained so installs that omit deprecated v1 packages do not hit broken read_skill links.
writing-relayflows (1.6.1) and relay-80-100-workflow (2.1.1) no longer point readers at writing-agent-relay-workflows or migrating-persona-to-relayflow. Legacy @relayflows/core warnings stay in-doc and explicitly say not to use that engine for new work. Matching copy updates land in README.md and prpm.json.
connecting-agents-across-machines (1.0.3) adds a process-list limitation callout before the human-only workspace join: CLI 13.x needs a positional key, so the key can briefly appear to local process-list readers even with the hidden-prompt flow—trusted single-user machines only, not from agent tools.
